Forms for Supervisors These forms must be submitted before any department can hire Federal Work Study (FWS) students. Submit all completed forms to FWS@hostos.cuny.edu. FWS Student Employee Request and Job Description Form Supervisors Rights & Responsibilities Forms for Students (requires printing & signatures) I-9 (requires signature & ID’s I-9 Form—Employment Eligibility Verification (PDF and Paper versions available) FWS earnings are considered taxable income and are treated as any other income when filling tax returns; therefore, student employees must also complete and sign the following forms: W-4 Form—Employee’s Withholding Allowance Certificate (requires signature) IT-2104 Form—Employee’s withholding Allowance Certificate (requires signature) IT- 2104E if applicable requires signature Notice and Acknowledgement of Pay Rate and Payday As of December 31, 2018 New York State's Minimum Wage is $15.00. Note: Students must have FICA taxes withheld if working at an off-campus agency/site and/or working during the Winter Session. *Documents must be submitted in-person to the Financial Aid Office*
